You Know Someone
Under Curatorship that Needs Glasses?
Here's how it works:
1_
Eye
Exam
The person under curatorship must see an optometrist for an eye exam. With the health insurance card, the basic examination is free for children 17 and under as well as for adults over 65. It might also be covered by the insurance company. After, you need to request a copy of your prescription. It’s very important!

Meeting with
An Optician
The person under curatorship must go to a point of service with his or her prescription and the name and contact information (preferably e-mail and telephone number) of the curator. Check out our schedule to find the closest service point. Read carefully, a lot of service points are appointment-only.

Request to the
Public Curator
The optician forwards the file and the order to a member of our team so that we can send the necessary authorization request.
4_
Payment and
Form
Once the application has been approved, the Public Curator will pay the fees and mail the MAS form (if applicable).

Picking Up
The Glasses
When the glasses are ready, we reach out to the tutor to inform him or her. The person under guardianship must be present when the glasses are picked up, since the frames will be adjusted and visual comfort checked at that time.
